The project aims to:

• Provide support services for people living in Chaucer ward, Rockingham Estate (a very large estate near to the Elephant and Castle), who have been diagnosed with chronic conditions such as diabetes, hypertension, cancer, Stroke, CHD, sickle cell, thalasseamia and HIV/AIDS.
• Enable people to become more involved in managing their conditions
• prevent, reduce or delay the onset of disabling complications of chronic diseases by providing appropriate education, information, advice and support and reinforcing the benefits of treatment/medicine compliance and a healthy life-style.
• This is Guys Charity supported project
